These questions will be presented on separate pages to facilitate your ability to save your work without loss of data for the awards process. You can save each page at anytime by going to the next question. Below are the questions that you will find on the proceeding pages that will require complete answers:
- Describe the challenge or problem you/your system experienced that motivated change or improvement. Please also identify the personnel, departments, any suppliers, distributors or GPO's involved in this process.
- What was the initiative you implemented? When did you start the implementation process? How did those involved impact the desired results? Who was involved in the review process? Please identify by listing individual names and appropriate teams.
- Identify any challenges and how you overcame those challenges that you encountered during the improvement process.
- What have been the tangible results thus far? Please provide estimates of future results. Please identify how you determined and measured your results.
